Paul is more than just an administrator. He is one of the owners of Suffolk Center and its Chief Operating Officer. So when you hear people say that nothing goes on at Suffolk Center without Paul knowing, you can believe it! And that’s one big reason why the facility runs so well. A sharp eye for the big picture and equally sharp attention to even the smallest details, along with an unbending commitment to the comfort and care of his residents, is Paul’s recipe for success. Paul’s leadership by example has spread that same dedication throughout the facility where Paul and his staff view taking care of their residents as a privilege.
Crystal Nevins RN
Director of Nursing
I was born and raised in Patchogue, New York. I have a BSN in nursing and continue to pursue my nursing education, graduating from LIU in May 2020 with High Honors as a Family Nurse Practitioner. Suffolk Center is a small facility, and it is very dear to me. My mother retired from her nursing career at the facility after 20+ years. I started my career at Suffolk Center in 2000 as a recreation aide. During that time, I was enrolled in school to follow my dreams to become a nurse. I worked at the facility as a nursing aide for a short time while I was waiting to take my LPN certification exam. I left the facility in March 2003, shortly after accepting a position in another local facility. In January 2019, I returned to the facility as the Director of Nursing. Suffolk Center has a dedicated team of caring professionals. It is our obligation and duty to provide safe, effective and quality care to each resident who enters our facility. We foster a very caring environment not only to the resident but to their family as well. My goal is to help make this Center a magnificent facility where every patient wants to go. We will plan to make a lasting impression for our residents and their families. We have many patients who, after being discharged, have setbacks and need rehab again; they only want to return to Suffolk Center. We will continue to strive to be the best facility.
Melissa Springer OTA
Director of Rehabilitation
Melissa has worked in Skilled Nursing Facilities for the last 15 years. Melissa joined Suffolk Center family in 2009 as an Occupational Therapist Assistant, bringing with her over ten years’ experience in skilled nursing facilities. Her winning way with residents complemented by her exceptional organizational skills earned her a promotion to Assistant Director of Rehabilitation in 2013. In September of 2015, she became Director of Rehabilitation where she leads an outstanding department of Occupational, Physical, and Speech Therapists. Everyone on Melissa’s team truly connects with every resident. They make it a point to get to know every patient in the facility personally as well as knowing their physical condition. According to Melissa, “The better we know our residents, the better job we can do of creating their personal care plans. And that leads to good outcomes in the therapy department, and everywhere else here at Suffolk Center.” With a first-class team of therapists, a state of the art facility, and Suffolk’s new rehabilitation car, this is one therapy department that can’t be beat.
Leta Agnihoti LMSW
Director of Social Services
Leta has been in the field of social work for over seventeen years and has been involved at every level from psychotherapy and psychiatric hospitals to nursing homes, assisted living facilities and adult day care. So it’s safe to say that there aren’t too many situations she hasn’t helped people deal with. “I’m so fortunate to have such a diverse background. We‘re here for our residents through every stage of their treatment, from welcoming them to Suffolk Center and getting them settled in, to preparing them for discharge. And thanks to my background, when I’m putting a family member or a resident at ease about almost anything, I can tell them that it’s something we’ve done before and not to worry. And that makes us both feel great.”
